Here's a complete breakdown of every Medicare cost you need to know, from premiums and deductibles to out-of-pocket maximums and how to reduce what you pay.

## The Real Cost of Medicare in 2026: A Complete Breakdown

Medicare is a federal health insurance program for Americans 65 and older, as well as some younger people with disabilities. While many people assume it's completely free because they've paid into it through payroll taxes their entire working lives, the reality is more nuanced. Let's break down every cost so you can plan your retirement healthcare budget with confidence.

$0 Part A Premium (Most People) 

$185/mo Part B Premium 

$1,650 Part A Deductible per Benefit Period 

## Part A: Hospital Insurance Costs

**Medicare Part A** covers inpatient hospital stays, skilled nursing facility care, hospice care, and some home health care. Here's what most people don't realize about Part A costs:

### Part A Premium

If you or your spouse worked and paid Medicare taxes for at least **10 years (40 quarters)**, you pay **$0 per month** for Part A. This is what people mean when they say "Medicare is free." But approximately 1% of Medicare beneficiaries haven't worked long enough and must pay:

-   **30-39 quarters of work:** $285/month in 2026
-   **Under 30 quarters:** $518/month in 2026

### Part A Deductible and Coinsurance

Even with a $0 premium, Part A has significant cost-sharing:

-   **Inpatient deductible:** $1,650 per benefit period (not per year — you could pay this multiple times)
-   **Days 1-60:** $0 coinsurance after deductible
-   **Days 61-90:** $413/day coinsurance
-   **Days 91-150 (lifetime reserve days):** $826/day coinsurance
-   **Beyond 150 days:** You pay 100% of costs

A single extended hospital stay could cost you **$10,000 or more** in coinsurance alone. This is a major reason Medicare Supplement (Medigap) plans exist.

## Part B: Medical Insurance Costs

**Medicare Part B** covers doctor visits, outpatient care, preventive services, durable medical equipment, and ambulance services. This is the part of Medicare that most people pay for monthly.

### 2026 Part B Premium

The standard Part B premium for 2026 is **$185.00 per month**. This is automatically deducted from your Social Security check. If you're not yet collecting Social Security, you'll receive a quarterly bill.

### Part B Deductible and Coinsurance

-   **Annual deductible:** $283 per year
-   **Coinsurance:** You pay 20% of the Medicare-approved amount for most services
-   **No out-of-pocket maximum:** Original Medicare has NO cap on what you pay out of pocket

That last point is critical. If you have a major surgery or cancer treatment, your 20% coinsurance on a $200,000 hospital bill would be **$40,000** — with no limit. This unlimited exposure is the #1 reason we recommend Medicare Supplement insurance.

## Part D: Prescription Drug Costs

**Medicare Part D** covers prescription drugs. You must join a separate Part D plan or get drug coverage through a Medicare Advantage plan.

### Average Part D Costs in 2026

-   **Average monthly premium:** $40-$55/month (varies by plan)
-   **Annual deductible:** Up to $590 (many plans have $0 deductible)
-   **Copays:** Vary by drug tier (generic vs. brand-name vs. specialty)
-   **Coverage gap ("donut hole"):** Largely eliminated in 2026 thanks to the Inflation Reduction Act
-   **Annual out-of-pocket cap:** $2,000 maximum in 2026

The $2,000 annual cap on Part D spending is a **game-changer for 2026**. Previously, Medicare beneficiaries could face tens of thousands in drug costs. Now, once you hit $2,000 in out-of-pocket drug spending, you pay $0 for the rest of the year.

## IRMAA: The Medicare Surcharge for Higher Earners

If your income exceeds certain thresholds, you'll pay more for Part B and Part D through the **Income-Related Monthly Adjustment Amount (IRMAA)**. This is based on your tax return from 2 years ago (2024 income determines 2026 IRMAA).

## Medicare Supplement (Medigap) Costs

A **Medigap plan** is private insurance that helps cover the gaps in Original Medicare — deductibles, coinsurance, and copayments. In Alabama, the most popular plans are:

-   **Plan G:** Covers everything except the Part B deductible ($283/year). Premiums typically range from $120-$200/month depending on age, gender, and tobacco use.
-   **Plan N:** Similar to Plan G but with small copays at doctor visits ($20) and ER visits ($50). Premiums are usually $80-$150/month.
-   **High-Deductible Plan G:** Lower premiums but you pay a $2,870 deductible first. Good for healthy seniors who want catastrophic protection.

What if I can't afford Part B?
